Our home care services include specialized post-op care from cardiac surgery, as well as disease management for conditions like congestive heart failure, COPD, pneumonia, and other cardiopulmonary diseases.

Catholic Health’s own infusion pharmacy provides compassionate care for patients receiving IV therapy services.

Nurse-Family Partnership

Nurse-Family Partnership creates a unique support system for first-time mothers, with the belief that our services create a better future for both mom and baby. Our program fosters trust between participating mothers and our trained nurses, who provide assistance throughout pregnancy, and up until the child’s second birthday.

Mother & Baby Home Care

Our home care nurses are passionate about providing comprehensive home care to both mom and baby, especially in cases of high-risk postpartum or if your baby requires extra care.

Palliative Care

Palliative care provides medical care and relief from the pain and stress of a chronic or terminal illness while receiving qualified skilled services. Palliative care at home can reduce hospital visits by bringing your nurses and doctors to you when you are ill but it’s not an emergency.

Starting Off Right Program

The Starting off Right program is here to provide support to mother and baby at no additional cost. This is a Catholic Health home based program that works in conjunction with Independent Health. We are here to support by providing assistance from pregnancy through the first 6 months after the birth of the child.

Home & Community Based Care Services

Catholic Health provides a various of home care services to those in Erie and Niagara Counties under the Home & Community Based Care umbrella – McAuley Seton Home Care, Mercy Home Care, and the Catholic Health Infusion Pharmacy.

McAuley-Seton Home Care is a certified home health agency that provides skilled nursing, physical therapy, occupational therapy, speech therapy, social work and home health aide services to patients who are recovering from a hospital stay, injury or illness, or require regular assistance with a medical condition.

Mercy Home Care is a licesnsed home care agency that provides personal care assistance, including assistance with daily living activities, as well as telehealth and home response services.

Catholic Health Infusion Pharmacy provides home infusion, retail and oral pharmacy services.
