Nurse-Family Partnership is a free resource that helps first-time mothers create a better future for themselves and their baby.

In this program, specially trained nurses schedule regular visits that start early in your pregnancy and continue through the child’s second birthday. This timeline allows new mothers to develop a close relationship with their nurses, who become a trusted resource and support system over time.

For women who meet income requirements, this program was designed to anticipate the barriers facing expectant mothers. The goals of the program are to keep children healthy and safe, while providing support that helps women become confident, successful mothers.

Frequently Asked Questions

Eligibility for Nurse-Family Partnership is determined if you are a woman who is:

  • Pregnant with your first child
  • Less than 28 weeks pregnant
  • Eligible for WIC or Medicaid
  • Resident of Erie or Niagara County

We ask that you contact us as early as possible, typically before week 28 of your pregnancy, so that we can help you prepare for childbirth and support you as you go through the challenge of becoming a parent for the first time.

The purpose of Nurse-Family Partnership is to build a support network for you and your baby. Our nurses are there to answer your questions, guide you in becoming self-sufficient, and connecting you with valuable community resources.

You will:

  • Prepare for the emotional, social, and physical changes you may experience throughout and immediately following your pregnancy
  • Learn about the birth process and know what to expect
  • Get help finding prenatal care, if needed
  • Learn how to modify your diet
  • Get help with any dependency on cigarettes, alcohol, or drugs
  • Develop a vision for your future, which may include staying in school, finding employment, or planning future pregnancies

Appointments are available during the day, Monday through Friday. Nurses make every effort to work with your schedule.

Home visit schedule:

  • Weekly for the first 4 weeks
  • Every other week until the child is born
  • Weekly for 6 weeks
  • Every other week until the child is 21 months old
  • Monthly until the child turns 2

We encourage fathers, family members, and friends to attend your home care visits and learn how they can support you. You can decide with your nurse who should be involved.
