Leader in Rehabilitative & Home Care

Inevitably, we all begin to require more care as we age. The pursuit of a healthy life suddenly seems more important than ever – as medical issues become more prominent, risks at home may develop, and awareness of our abilities may decline.

Senior care at Catholic Health is provided on either a long- or short-term basis, by a multi-disciplinary team that includes doctors, nurses, therapists, and home health aides. Whether you elect yourself or your loved one to receive care in a rehab center or at home, a coordinated approach guides all of our treatment decisions.

At Catholic Health, we strive to provide you with compassionate, cost-efficient care. Our programs are nationally recognized for quality and excellence by the Centers for Medicare and Medicaid, as well as HomeCare Elite, which rates home health agencies.

Senior Support Services

Palliative Care

Palliative care helps with the management of a chronic or terminal illness by improving quality of life for both the patient and their loved ones. Rather than traveling to a hospital, palliative care services bring nurses and doctors to you, providing pain relief and additional support during difficult times.

Medical Alert System

Catholic Health home response is your direct link to emergency help 24/7 – whether at home or on the go. With just the push of a button, you’ll instantly be connected to one of our highly-trained emergency response specialists.

Assistance with Daily Living Activities

Maintaining independence becomes more important as we age, or perhaps lose some mobility. Nursing aides also provide assistance with daily living activities – those essential skills that satisfy our most basic needs. From housekeeping tasks to personal grooming habits and daily companionship, we’re there to support you when you need it most in your life.

Spiritual Care

No one can prepare for how you might react when you or a loved one experiences a medical emergency or potentially life-threatening situation. Our spiritual care team is on-call 24/7 to provide the right kind of support for you – through discussion, prayer, and more.
