The Health Home Care Management program at Catholic Health helps ensure that Western New Yorkers with chronic health concerns, who receive health benefits through Medicaid have access to the care providers they need. It can be a struggle for many to find services that accept Medicaid, which makes it difficult for people to recover and maintain their health.

The goal of our Health Home program is to make care accessible to these patients, allowing them to decrease their trips to the emergency room and receive consistent, quality care in their community.

What is a Health Home program?

The Health Home program at Catholic Health is a comprehensive group of healthcare and service providers that work within our healthcare system, providing coordinated care to active Medicaid recipients. Once enrolled in the program, you will have access to a care manager that will work with you to develop a care plan to fit your needs. This will map out the services to treat your existing conditions and outline what you need to maintain better health.

Provided services may include:

  • Coordinating care between your healthcare providers
  • Access to mental health and substance abuse providers
  • Managed medications
  • Assistance with housing
  • Social services (such as food, benefits, and/or transportation)
  • Connecting with community resources

Who is Eligible for the Health Home Program?

  • Active Medicaid recipients with a primary care physician
  • Two chronic health conditions (asthma, diabetes, heart disease, COPD, obesity, substance abuse disorder, etc.)*
    – or –
    A single qualifying diagnosis, such as a serious mental health condition (bipolar disorder, schizophrenia, etc.) or immunodeficiency like HIV/AIDS
    – and –
  • Has significant behavioral, medical or social risk factors that can be addressed through care management
