McAuley Seton Home Care

Catholic Health's home care services are provided by McAuley Seton Home Care and Mercy Home Care, and are available to residents of Erie County and Niagara County.

Skilled Nursing for Seniors

Skilled nursing is the highest level of care a patient can receive outside of a hospital. Registered nurses, licensed practical nurses, and other healthcare professionals provide 24/7 care for seniors who are recovering from an illness, injury, or living with a chronic condition. 

Assistance with Daily Living

The nursing aides in our home care programs provide assistance with daily living activities, or those essential skills that satisfy our most basic needs. From housekeeping tasks to personal grooming habits and daily companionship, we're there to support you when you need it most in your life. 

Cardiopulmonary 

The expert services of our specialty cardiac team are available to eligible home care patients. We provide post-operative care, disease management, and additional support services to better position patients for a successful recovery. Cardiac patients may require supplementary care from physical therapists and relevant specialties. 

Nurse-Family Partnership

Nurse-Family Partnership creates a unique support system for first-time mothers, with the belief that our services create a better future for both mom and baby. Our program fosters trust between participating mothers and our trained nurses, who provide assistance throughout pregnancy, and up until the child's second birthday. 

Home Rehab Therapy

Specialized rehab services offered in the comfort of the patient's home include physical therapy, wound therapy, speech therapy, and services of licensed social workers and registered dietitians. 

Infusion Therapy

Catholic Health's own infusion pharmacy provides compassionate care for patients receiving IV therapy services. 

Mother and Baby Home Care

Certain circumstances can make delivery or the postpartum period especially difficult for some mothers. Mother and baby home care nurses are dedicated to going the extra mile in the way that some at-risk pregnancies may require. The number of visits that you will receive varies depending on insurance coverage and condition of mom and baby. 

Spiritual Care

No one can prepare for how you might react when you or a loved one experiences a medical emergency or potentially life-threatening situation. Our spiritual care team is on-call 24/7 to provide the right kind of support for you – through discussion, prayer, and more.

Palliative Care

Palliative care provides medical care and relief from the pain and stress of a chronic or terminal illness while receiving qualified skilled services. Palliative care at home can reduce hospital visits by bringing your nurses and doctors to you when you are ill but it's not an emergency.
