Programs for Senior Care

Program of All-inclusive Care for the Elderly (PACE)

LIFE – Living Independently for Elders – is Catholic Health's PACE program. With activities that present both recreational and social opportunities for participants, this is an adult care program for people that still live at home, but may be in need of more supervised care. 

Skilled Nursing for Seniors

Skilled nursing is the highest level of care a patient can receive outside of a hospital. Registered nurses, licensed practical nurses, and other healthcare professionals provide 24/7 care for seniors who are recovering from an illness, injury, or living with a chronic condition. 

Home Care

For seniors who still live at home, Catholic Health's home and community-based care enables participants to receive help managing their care, while still maintaining independence. 

Medical Adult Day Care

The St. Catherine Adult Day Care Program allows individuals to live at home and enjoy a social network that can eliminate feelings of isolation. It can also reduce the stress that family, friends, and caregivers may experience in managing an individual's needs. 

Infusion Therapy

Catholic Health's own infusion pharmacy provides compassionate care for patients receiving IV therapy services either at home or in a skilled nursing facility.

Palliative care helps with the management of a chronic or terminal illness by improving quality of life for both the patient and their loved ones.     The rehabilitation center at McAuley Residence on the Kenmore Mercy Hospital campus is the only orthopedic rehab unit in Western New York dedicated to helping you regain mobility after a knee or hip replacement.

  • Mercy Nursing Facility at OLV

    Residents in this Lackawanna care facility each have a private room and live in a household model. This allows patients to maintain their independence while receiving the medical care they need, including rehabilitation, medication management, and more.

  • Father Baker Manor

    The rehab experts at Father Baker in Orchard Park work with you after a surgery or health event, such as a heart attack or stroke. We get you back up to speed and help you return to regular activities. Our staff coordinates your rehab and assists with residential needs. 

  • St. Catherine Labouré Health Care Center

    The care team at St. Catherine Labouré Health Care Center offers specialized treatment in long- and short- term rehabilitation. Conveniently located on the Sisters of Charity Hospital campus on Main Street in Buffalo, this facility provides individualized senior care. 

  • St. Francis Park

    Opened in 2018, St. Francis Park provides a unique independent living option for seniors aged 65 and older. Beautiful studio apartments are a place to call home for residents who may be looking to downsize or lessen the burdens of home maintenance.
