Programs for Senior Care
Program of All-inclusive Care for the Elderly (PACE)
LIFE – Living Independently for Elders – is Catholic Health's PACE program. With activities that present both recreational and social opportunities for participants, this is an adult care program for people that still live at home, but may be in need of more supervised care.
Skilled Nursing for Seniors
Skilled nursing is the highest level of care a patient can receive outside of a hospital. Registered nurses, licensed practical nurses, and other healthcare professionals provide 24/7 care for seniors who are recovering from an illness, injury, or living with a chronic condition.
For seniors who still live at home, Catholic Health's home and community-based care enables participants to receive help managing their care, while still maintaining independence.
Medical Adult Day Care
The St. Catherine Adult Day Care Program allows individuals to live at home and enjoy a social network that can eliminate feelings of isolation. It can also reduce the stress that family, friends, and caregivers may experience in managing an individual's needs.
Catholic Health's own infusion pharmacy provides compassionate care for patients receiving IV therapy services either at home or in a skilled nursing facility.
Senior Support Services
Support services available for seniors and their loved ones include:
Palliative care helps with the management of a chronic or terminal illness by improving quality of life for both the patient and their loved ones. Rather than traveling to a hospital, palliative care services bring nurses and doctors to you, providing pain relief and additional support during difficult times.

Medical Alert System
Catholic Health home response is your direct link to emergency help 24/7 – whether at home or on the go. With just the push of a button, you'll instantly be connected to one of our highly-trained emergency response specialists.
Assistance with Daily Living
Maintaining independence becomes more important as we age, or perhaps lose some mobility. Nursing aides also provide assistance with daily living activities – those essential skills that satisfy our most basic needs. From housekeeping tasks to personal grooming habits and daily companionship, we're there to support you when you need it most in your life.
No one can prepare for how you might react when you or a loved one experiences a medical emergency or potentially life-threatening situation. Our spiritual care team is on-call 24/7 to provide the right kind of support for you – through discussion, prayer, and more.
