LIFE Day Center at D’Youville is part of Catholic Health’s PACE program, or Program of All-inclusive Care for the Elderly. Located on the D’Youville Campus in Buffalo, this newly renovated facility provides seniors with the medical they need along with daily social activities to enhance their quality of life.

Our LIFE program provides better access to doctors and caregivers, a range of social activities and free transportation to our center, as well as any needed doctor’s appointments.

Services

Program All-Inclusive Care for the Elderly (PACE) – Catholic Health’s PACE program is known as LIFE (Living Independently for Elders), and is available to those who are dually eligible for Medicare and Medicaid. This program coordinates and manages a care plan for seniors that may include rehab, primary care, transportation, medication, and more.

Services at LIFE D’Youville include:

• Physician services
• Nursing
• Rehabilitation therapies (physical, occupational and speech therapies)
• Social work services
• Spiritual care
• Nutritional services
• Personal care, such as showering or bathing
• Recreational and educational activities
• Round-trip transportation to and from the center

Parking

Free parking is available for patients and visitors of LIFE D’Youville in the lot next to our facility.
