All items in this checklist are needed to rotate at any Catholic Health System hospital, unless otherwise noted.

  1. ID Badge & Parking (Sisters Hospital only) – Complete Student Access Form
    • Refer to this parking map, you should park in the Associate Parking (AP) Lot
  2. ID Badge & Parking (Mercy Hospital only) – Complete Catholic Health System ID Authorization Form (please contact the Program Administrator at 716-862-1423 for more information).
  3. Student/Resident Attestation Form
  4. EMR ‐ Soarian Access will be set up by the coordinator; however see the attached sheet for password tips.
