The Catholic Health Referral Support and Transfer Center is located on the third floor of Mercy Hospital of Buffalo. It offers a one-call system for transfers and direct admissions into the Catholic Health Acute Care System.

Process Checklist

For reference, follow these easy steps to transfer or direct admit your patient into any Catholic Health Acute Care facility.

  • Call (716) 374-4094
  • Provide patient’s name and date of birth, clinical reason for transfer, diagnosis, requested level of care, placement preference, and primary care provider.
  • If you are in need of an admitting physician, you will be asked to remain on the phone to be directly connected with an admitting hospitalist, facilitating physician hand off.
  • Once an admitting physician is verified, the Transfer Center will advise you of an admission room number, or develop an acceptable transfer or direct admission plan with you.
  • For all external transfers being admitted into a Catholic Health acute care site, you will be asked to transfer the call to the Registered Nurse caring for the patient to coordinate nursing report and transfer arrangements.
