How to download the CH MyChart App

To download the CH MyChart app, please follow the steps below:

• In the app store (Google Play or the Apple App Store) search for the CH MyChart app.
• Download the app to your device
• After the download is complete, open the app, and log in using your Catholic MyChart username and password. If you do not have a username and password, select Sign up now to set up an account.
