Catholic Health home care services make life a little easier by bringing our trademark compassionate care to you.

When you receive home care, a nurse or aide will visit you at home to provide medical care or other services. Home Care is generally recommended if:

  • you have been discharged from a hospital or other facility but require additional assistance at home
  • you have medical conditions or physical challenges
  • you need assistance with personal care, household chores or other activities of daily living

Services

Skilled Nursing  – Skilled nursing is the highest level of care a patient can receive outside of a hospital. Registered nurses, licensed practical nurses, and other healthcare professionals provide 24/7 care for patients who are recovering from an injury, illness, or living with a chronic condition. Learn More >

Home Rehab Therapy – Specialized rehab services at home include physical therapy, occupational therapy, speech therapy, and treatment from registered dietitians, licensed clinical social workers, and certified wound therapists. Learn More > 

Assistance with Daily Living Activities – Daily living activities are those essential skills that satisfy our most basic needs. On-site surveys are conducted every three years by industry experts. A comprehensive review is conducted of organizational structure, policies & procedures, compliance with federal/state/local laws, leadership, patients’ rights & responsibilities, fiscal operations, human resource management, provision of care, patient records, quality outcomes, performance improvement, infection control and patient/employee safety.
