Catholic Health’s Home & Community Based Care provides a comprehensive range of health and supportive services designed to help individuals maintain their well-being and continue living safely and comfortably in their own homes or community long-term settings.

Our care team coordinates personalized services based on each person’s unique health needs, ensuring they receive the right support to recover, manage ongoing conditions, and/or maintain overall wellness.

Skilled Nursing  – Skilled nursing is the highest level of care a patient can receive outside of a hospital. Registered nurses, licensed practical nurses, and other healthcare professionals provide 24/7 care for patients who are recovering from an injury, illness, or living with a chronic condition. Learn More >

Home Rehab Therapy – Specialized rehab services at home include physical therapy, occupational therapy, speech therapy, and treatment from registered dietitians, licensed clinical social workers, and certified wound therapists. Learn More > 

Assistance with Daily Living Activities – Daily living activities are those essential skills that satisfy our most basic needs. The nursing aids in our home care program provide support where you need it most in life, from housekeeping tasks to personal grooming habits and daily companionship. Learn More > 

LIFE (A PACE Program) – LIFE is an adult care program for participants that still live at home, but may also be in need of more supervised care. Our services promote a healthy, engaging lifestyle for adults – the right kind of care, made available at just the right time. Learn More > 

Cardiopulmonary – Recovering from a cardiac event or procedure requires specialized rehabilitation efforts. Catholic Health offers our patients access to cardiac rehab services in two different ways: as an outpatient program, or as part of our home and community-based care efforts. Learn More >

Updated quarterly, they recognize agencies that consistently deliver exceptional care and service across every touch point.

U.S News & World Report
Father Baker Manor and Mercy Nursing Facility at OLV were both recognized as “High Performing” by U.S. News & World Report for long-term care in 2026. Father Baker Manor was also listed as “High Performing” in Short-term Rehabilitation. U.S. News & World Report is a leading national authority on hospital rankings and consumer advice. The organization says out of the nearly 15,000 U.S. nursing homes, fewer than 19% get recognized as “Best Nursing Homes” in “Long-Term Care” or “Short-Term Rehabilitation.

Newsweek Recognizes Mercy Nursing Facility, Father Baker Manor & McAuley Residence in Best Nursing Homes 2025 Survey
Mercy Nursing Facility at OLV ranked number four in New York State in nursing homes with 50-99 beds. It was only one of five facilities in New York to make the list in this category. For nursing homes with 150+ beds, Father Baker Manor ranked number 35, and McAuley Residence ranked number 50 in New York State. Newsweek’s Best Nursing Homes 2025 List included 1,050 facilities across 35 states with the highest number of nursing homes according to Centers for Medicare & Medicaid Services (CMS) data.

Mercy Nursing Facility at OLV Named “Top Performing Nursing Home” by New York State Department of Health – 2024
Mercy Nursing Facility at OLV was named a “Top Performing Nursing Home” by the New York State Department of Health’s Nursing Home Quality Initiative (NHQI). Only 40 nursing homes out of the more than 600 in New York State received this designation. The NHQI is an annual quality an performance evaluation project designed to improve care to residents in Medicaid-certified nursing homes across New York State.

Our services may be covered by Medicare, Medicaid, HMOs, private health insurance, or on a private-pay basis.
