Catholic Health System Continuing Care Foundation supports Catholic Health's LIFE/PACE and Home Care ministries. The foundation raises funds to help provide our seniors with services to assist our aging population in maintaining independence and care at home.

Ways to Give
If there's a physician, nurse, or caregiver who has made a meaningful impact on your life or the life of a loved one, please consider sending them a note of gratitude by filling out our form and making a gift to the Continuing Care Foundation in their honor.
You may also consider making a donation in honor of a doctor or caregiver who is especially meaningful to you. While the amount of your gift remains confidential, your Angel will be notified that you made a gift in his/her honor, and they will receive an Angel pin to wear. Your secure credit card donation can be made through our online donation form.
