Nursing Home Care

McAuley Residence is a spacious, air-conditioned nursing home conveniently located on the same campus as Kenmore Mercy Hospital.

We provide skilled nursing services and assistance with activities of daily living, such as bathing, dressing or eating.

Becoming a Resident

To become a resident, download an application at the link to the left and submit it to CareConnection by faxing it to (716) 923-4894 or by mailing it to the following address:

144 Genesee Street
5th Floor
Buffalo, NY 14203

Subacute Rehabilitation

Subacute care is primarily for patients who have recently been hospitalized and need a transitional care setting between their hospital stay and their return to home.

Patients enrolled in a subacute program reside for a short period (28 days, on average) at McAuley Residence, where they receive nursing and therapy care to rebuild their strength and also learn how to better manage their disease or condition

Our subacute services are provided on dedicated subacute units. You will be with other patients who may have similar conditions. You will also have access to an expansive gym area conducive to your quick and safe recovery.

Becoming a Subacute Patient

Patients are often discharged from a hospital to a subacute setting and are referred by a physician, case manager or discharge planner. Patients and their families can request a Catholic Health subacute facility by telling their hospital discharge planner of their preference.
