Ease of Referrals Catholic Health Senior Services has a streamlined admissions process, allowing for easy referrals to our senior services programs. Referrals are accepted from physicians, Catholic Health facilities, family members and community health service organizations. Community case managers at hospitals coordinate a patient’s referral to our senior services programs through the Catholic Health Care Connection, (716) 706-2382. This number is equipped with a voice mail system in the event Central Intake coordinators are servicing other clients at the time of your call. Multiple Payment and Insurance Options Several payment options are available, including private pay, Medicare and Medicaid. Financial counselors will help guide patients and their caregivers through this process at the time of referral/admission. In general, Medicare will pay for short periods of skilled nursing facility care for eligible patients who have been hospitalized for at least three days and need continued care. Medicaid will pay for long nursing home stays for those whose income and savings are insufficient to cover the cost. Subacute rehabilitation services are covered by most insurance plans including Medicare, Medicaid, Workers’ Compensation, No Fault, Blue Cross/Blue Shield, Community Blue, Univera and Independent Health. Medical adult day healthcare, including transportation, is usually covered by Medicaid. A private pay option is also available for this service.
