Making Your Appointment
Private nutritional counseling services are available by appointment. Your session will take place in a quiet and confidential atmosphere where you will have your questions addressed by a qualified dietitian.
Before You Make Your Appointment: To receive counseling, you must have a prescription or referral from your doctor indicating your diet order and diagnosis code. Please contact your doctor for a prescription before making your appointment.
What to Bring: On the day of your appointment, bring your prescription, insurance card and photo ID.
Many medical insurance plans provide coverage for outpatient nutrition services.
Prior to scheduling your appointment, please check with your health insurance plan to determine if you are fully covered or if prior approval is required.
