Private Nutrition Counseling

According to the U.S. Surgeon General's Report on Nutrition and Health, eight of the ten leading causes of death, including coronary heart disease, stroke, some types of cancer, and diabetes are related to diet.

Nutrition counseling is a cost-effective way to treat and control preventable diseases and medical conditions, including diabetes.

How to Make an Appointment

Private nutritional counseling services are available by appointment. Your session will take place in a quiet and confidential atmosphere where you will have your questions addressed by a qualified dietitian.

Before You Make Your Appointment: 

  • To receive counseling, you must have a prescription or referral from your doctor indicating your diet order and diagnosis code. Please contact your doctor before making your appointment.
  • Check with your insurance carrier to learn if it will cover the cost. If your insurance plan does not cover this service, you will be billed.

What to Bring to Your Appointment

  • Your prescription
  • Your insurance card
  • Photo ID

Please arrive 15 minutes prior to your appointment.
