Ann Marie Smokowski, RD, CDE, CDN, teaches a diabetes class.

Ann Marie Smokowski, RD, CDE, CDN has been employed by Mercy Hospital of Buffalo since 1984. She is a registered dietitian, certified diabetes educator and certified dietitian/nutritionist. She teaches diabetes education classes and is the Outpatient Medical Nutrition Therapy Counselor. Ann Marie takes pride in helping clients reach their nutritional goals.

I have lost a total of 81 lbs. since the diabetes class.

My diabetes was diagnosed early and with the early intervention, lifestyle changes including walking one mile daily, following my meal plan consistently, and watching portions, I no longer require my oral Diabetes medication! The class instruction (especially the meal plan) taught me to change my lifestyle for the better. I feel great!

Appointments

How to schedule an appointment for outpatient nutrition counseling at Mercy Hospital in the Marian Building:

  • Get a script or referral from your physician indicating your diet order and diagnostic code
  • Call (716) 828-2101 to schedule your appointment.
  • Check with your insurance carrier regarding your coverage (if service is not covered by insurance, payment in full is due at time of registration).

On the Day of Your Appointment

The Dietitian’s office is located in the Marian Professional Building at 515 Abbott Road, Suite 102A.

You must register on the first floor of the Marian Building, Suite 102A prior to your appointment. Please arrive approximately 10-15 minutes early. Do not go to the lower level.

Remember to bring your insurance card to your appointment.

Diabetes Education Classes

Diabetes classes are taught in four parts. One part is taught by Ann Marie and the remaining three are taught by certified diabetes educators.

Persons meeting the following criteria should attend a diabetes education class:

  • Recently diagnosed with diabetes
  • Had a change in treatment or nutrition plan
  • Has not had a previous opportunity to attend class
  • Has difficulty controlling their diabetes
