Short-term home care is provided to individuals who are expected to need home care for a limited time, such as when returning from a hospital or other healthcare facility.
Catholic Health provides short-term home care to patients through our McAuley Seton Home Care division.
Patients may receive any combination of services that meets their individual needs. The appropriate type and amount of service provided is determined by the patient, physician and McAuley Seton’s professional healthcare team.
Nursing Care
Infusion Therapy and Pain Management
Management of Cardiac and Pulmonary Diseases
We work with your primary physician to manage health challenges related to cardiac and pulmonary diseases and avoid emergency room treatment and possible hospitalization. With the use of our Congestive Heart Failure Telemanagement Program, daily monitoring of key vital signs can continue for up to 12 months after home care discharge.
Rehabilitation
Palliative care focuses on providing relief from the pain, stress and other debilitating symptoms of terminal illnesses. Unlike hospice care, which focuses on comfort care, palliative care continues to provide treatment regardless of life expectancy.
Payment for services provided through McAuley Seton Home Care may be covered by Medicare, Medicaid, HMOs, private health insurance, or on a private-pay basis.
