Long-term Home Care

The “nursing home without walls” program provided by Sisters Long Term Home Health Care is a cost-effective alternative to nursing home placement for people living with chronic (long-lasting or reoccurring) conditions.

Becoming a Patient

To obtain long-term home care services, patients must:

• Reside in Erie County
• Have their physician’s approval with medical orders certifying the need for services
• Live in a home where healthcare needs can be adequately and safely met

Referrals may be made by:

• Physicians
• Discharge planners
• Patients
• Family members or friends concerned about the care and well-being of the individual needing care

Upon receiving a referral, an evaluation is scheduled by one of our experienced nurse case managers. This evaluation will determine the patient’s eligibility for long-term home healthcare services.

If the patient is determined to be eligible, a plan of care is established and appropriate services are arranged and coordinated by the professional healthcare team. On an on-going basis, the plan of care is assessed and services are increased or decreased, based on the patient’s needs.

Paying for Services

Payment for services provided by Sisters Long Term Home Health Care may be covered by Medicare, Medicaid, or on a private-pay basis.
