Home care provides healthcare and social services at the patient's home. It is appropriate for:

• Patients who have been discharged from a hospital or other facility but require additional assistance at home
• Individuals with medical conditions or physical challenges
• Individuals needing assistance with personal care, household chores or other activities of daily living

Catholic Health's home healthcare services, provided in the Buffalo and Western New York area, include:

• Short-term nursing and therapy services
• Long-term care as an alternative to nursing home placement
• Assistance with activities of daily living, such as personal care, housekeeping, running errands
• A home response alarm that delivers access to emergency care 24/7
• Spiritual care
