Care Connection

Catholic Health has made the process of navigating through multiple community service agencies and varying health insurance policies easier by streamlining patient access and referrals through Care Connection.

Catholic Health Care Connection collaborates with case management, physicians, insurance liaison staff and families to ensure that you receive the care you need at the right place and the right time.

Care Connection can:

act as your patient advocate, ensuring timely coordination of care
coordinate appropriate use of Catholic Health facilities and services
coordinate the transfer of care to such programs and services as:
- Rehabilitation Care – hospital-based medical rehabilitation units, outpatient rehabilitation, subacute/short-term care
- Long-Term Care – adult care facilities, medical adult day healthcare, skilled nursing facilities
- Home Care – short-term skilled care, long-term skilled care, private duty support, emergency response network
