With Catholic Health’s HeartStrong program, a short-term (subacute) rehab stay helps patients to rebuild their strength and endurance and learn how to better manage their diseases before they return home.
Patients enrolled in a subacute program reside for a short period (28 days, on average) at a designated facility where they receive nursing and therapy care to rebuild their strength and also learn how to better manage their disease or condition.
The outpatient cardiac rehabilitation program is a two-phase approach designed to meet the needs of individuals recovering from heart attack, bypass surgery or other forms of heart disease.
Patients participate in physician-supervised aerobic exercise and an education program that emphasizes lifestyle changes and healthy living. Patients exercise wearing a cardiac monitor for most or part of the program.
