Patients enrolled in a subacute program reside for a short period (28 days, on average) at a designated facility where they receive nursing and therapy care to rebuild their strength and also learn how to better manage their disease or condition. Outpatient RehabilitationThe outpatient cardiac rehabilitation program is a three-phase approach designed to meet the needs of individuals recovering from heart attack, bypass surgery or other forms of heart disease. Patients participate in physician-supervised aerobic exercise and an education program that emphasizes lifestyle changes and healthy living. Patients exercise wearing a cardiac monitor for most or part of the program. |
