Diabetes Management – Fee Applies

You are the most important member of the diabetes team! Daily activities affect blood sugar control. You decide what to eat, whether or not to exercise and when to take medication. Because the part you play is so important, you need to keep yourself informed with up-to-date information.

Diabetes Classes

Who Should Attend

Anyone who meets the following criteria should attend class:

  • Was recently diagnosed with diabetes
  • Has had a change in treatment or nutrition plan
  • Has not had a previous opportunity to attend class
  • Has difficulty controlling their diabetes 

Instructors

Classes will be taught by registered nurses and registered dietitians who are certified diabetes educators.

Class Times and Locations

Every program consists of a series of four classes. You have the option to choose a series offered in the morning, afternoon or evening. Check our Calendar of Events for a list of upcoming Diabetes classes.

Catholic Health offers several class sites, conveniently located throughout Western New York. Program sites are handicapped accessible.

  • Avoiding long-term complications

  • Goal setting and problem solving

  • Lifestyle changes to promote a healthy lifestyle

Important Points

  • Program charges are covered by most insurances. You should check with your insurance company prior to scheduling classes.
  • Your physician will be contacted to complete the required referral form. 
  • A family member or friend may attend with you at no additional cost.
  • If you require specific assistance or service, please notify HealthConnection when you register.

The Catholic Health Lifeskills Diabetes Education Program has been recognized by the American Diabetes Association for Quality Self-Management Education.

Additional Services

  • Individual instruction may be scheduled for insulin administration or home blood glucose testing (a prescription from your doctor is necessary for this service. Those with Medicare must have a physician referral).
  • "Continuing Your Journey," an optional follow-up session is available for those who attended the four-class series.
  • Gestational Diabetes class is available at some sites.
