If you would like to recognize a Catholic Health employee, physician or volunteer for providing outstanding care, please fill out the form below.

The person recognized will be nominated for the LOVE Award (Living Our Values Effectively), a quarterly award designed to recognize employees who demonstrate our values in their daily work.

Anyone can nominate an associate for the LOVE award – patients, visitors, family members, physicians, volunteers and co-workers.
