Catholic Health accepts most insurances. To learn if Catholic Health’s services are covered under your insurance plan, please contact your insurance provider.
To obtain a copy of your medical records, please fill out an authorization form and mail or fax it to the hospital at which you received care.
If you are requesting medical records from any other Catholic Health facility, please contact the hospital of which the facility is a department. For example, Mercy Comprehensive Care Center is a department of Mercy Hospital of Buffalo. If you are a patient at Mercy Comprehensive Care Center, requests for medical records should be submitted to Mercy Hospital of Buffalo.
To determine which hospital oversees the facility at which you received care, visit the facility section of the website and locate the page for the facility at which you received care. The information will be listed at the top of the page.
