The Catholic Health System has made the process of navigating through multiple community service agencies and varying health insurance policies easier by streamlining patient access and referrals through Care Connection.

One-Stop Solution

CHS Care Connection collaborates with case management, physicians, insurance liaison staff, and families. They are a valuable resource for information on community-based programs and services. Care Connection:

Makes sure that the quality of care meets desired outcomes
Is a proactive patient advocate, ensuring timely coordination of care
Coordinates the appropriate use of CHS facilities and services
Coordinates the transfer of care to such programs and services as:
- Rehabilitation Care - hospital-based medical rehabilitation units, outpatient rehabilitation, subacute/short-term care
- Long-Term Care - adult care facilities, medical adult day health care, skilled nursing facilities
- Home Care - short-term skilled care, long-term skilled care, private duty support, emergency response network
