Conveniently located in a stately residential area of North Buffalo just off Main Street, St. Francis of Buffalo is easily accessible by car or public transportation. Comprised of 120 skilled and short-term rehabilitation beds, as well as a medical adult day care program, St. Francis of Buffalo is close to all hospital services and amenities at Sisters of Charity Hospital. Residents and patients do not live at our facility, we work in their home. Our staff takes great pride in providing a homelike atmosphere for residents and patients, and ensuring that each is cared for with the utmost dignity and respect. Spirituality of all denominations is woven into the fabric of services provided. Our chaplain is expert in servicing the spiritual needs of residents and patients. Mass is celebrated weekly along with other denominational services in our Chapel.

Skilled Nursing/Long-Term Care
Through our well-rounded long-term care programs, we provide residents with comprehensive skilled nursing services and assistance with activities of daily living in safe, caring and supportive environments. Long-term care includes many social and recreational activities to help residents live as active, dignified and independently as possible. Our goal is to foster a sense of family and community for our residents.
Subacute/Short-Term Care
Subacute care is for patients of all ages who have recently been hospitalized and need a transitional care setting between their hospital stay and their return to home. Subacute care is provided by Partners In Rehab, the Catholic Health System’s rehabilitation division. Together, Partners In Rehab and St. Francis of Buffalo take an integrated approach to subacute care, incorporating several disciplines to meet each patient’s individual needs whether they be medical, rehabilitative, social or emotional. The goal is to help people coping with a medical condition achieve their highest possible level of function and self-sufficiency before returning home. Physicians may pre-certify elective surgical cases to expedite the admissions process.
Medical Adult Day Care
Adult day care is a community-based program that provides comprehensive healthcare services in a group day setting. The goal is to help each participant achieve and maintain the highest level of personal independence possible. Services provided, based on individual need, include:

- physical,occupational, and speech therapy
- nursing care
- audiology services
- nutritional counseling
- social services/care management
- daily activities
- lunch and pastoral/spiritual care services
