Subacute Care Facilities

Partners In Rehab's subacute facilities provide a bridge from hospital to home with an integrated approach to care. A multi-disciplinary team of physicians, nurses, and therapists meet each patient's individual needs whether they are medical, rehabilitative, social, or emotional. The goal is a quick and safe recovery. We want you to achieve your highest possible level of function and self-sufficiency before returning to your home or community.

Partners In Rehab provides subacute care in dedicated units within four of our Senior Services facilities. Patients may need assistance with recuperation after surgery, management of a chronic condition, or wound care.
