Continuing Care Division

Demands of the 21st century are transforming the face of health care. It is not just hospitals filled with doctors and nurses – it encompasses all the services a person needs to stay healthy and fit, no matter what their age. Through the Continuing Care division, experts are available in the areas of Rehabilitation and Senior Services at conveniently located facilities throughout Western New York.

At Partners In Rehab & AthletiCare, our vision is to provide a seamless continuum of rehabilitation from acute care to long-term care. We concentrate on the quality and value of rehabilitation services through outcome studies, best practices, and research. We are committed to promoting health and wellness throughout Western New York.

The Continuing Care’s Senior Services is one of the largest and most respected providers of care for seniors in WNY. We specialize in traditional skilled nursing/long-term care, subacute/short-term care, and medical adult day care. In addition, the Catholic Health System is taking health care into the 21st century with the Our Lady of Victory (OLV) Senior Neighborhood, a comprehensive, state-of-the-art living facility for senior citizens. Developed at the OLV Hospital campus, the OLV senior neighborhood will offer a range of senior housing options, a skilled nursing facility, and the Program for All-inclusive Care for the Elderly (PACE) program, complemented by on-campus shopping, a chapel, social activities, medical care, and other amenities.
