Be sure to include the signature of the person whose records are being requested. Please note that if records are requested from a Catholic Health System facility, there may be a fee involved. If the request is sent directly to your Medical Provider there should be no charge.
When making a request to a CHS facility for medical records, please address your request to the Correspondence Secretary in the Health Information Department at the appropriate location:

Obtaining Birth RecordsIf you were born in one of our hospitals, your birth records are kept in the Clerk's Office in the municipality in which you were born.
