Subacute care is primarily for patients who have recently been hospitalized and need a transitional care setting between their hospital stay and their return to home.
Patients enrolled in a subacute program reside for a short period (28 days, on average) at a designated facility where they receive nursing and therapy care to rebuild their strength and also learn how to better manage their disease or condition.

Patients are often discharged from a hospital to a subacute setting and are referred by a physician, case manager or discharge planner. Patients and their families can request a Catholic Health subacute facility by telling their hospital discharge planner of their preference.
