Conveniently located in the wooded village of Williamsville, St. Francis of Williamsville is minutes from the New York State Thruway, quaint boutique shopping and area malls, fine eateries and the ever-popular Glen Park.
Our staff takes great pride in providing a homelike atmosphere for patients, ensuring that each is cared for with the utmost dignity and respect.
Subacute care is for patients of all ages who have recently been hospitalized and need a transitional care setting between their hospital stay and their return to home. Subacute care is provided by Partners In Rehab, Catholic Health’s rehabilitation division.
Together, Partners In Rehab and St. Francis of Williamsville take an integrated approach to subacute care, incorporating several disciplines to meet each patient’s individual needs, whether they be medical, rehabilitative, social or emotional. The goal is to help people coping with a medical condition achieve their highest possible level of function and self-sufficiency before returning home. Physicians may pre-certify elective surgical cases to expedite the admissions process.
Quality care means achieving the best possible outcomes for our patients. We accomplish this through our team of interdisciplinary professionals with special training in caring for those with short-term rehabilitation needs. This team includes physicians, nurses, physical, occupational and speech therapists, pharmacists, dietitians, nutritionists, social workers, case managers, chaplain, activity directors, and patient and staff educators.
Our management staff is accessible 24 hours a day, 7 days a week.
Patients are often discharged from a hospital to a subacute setting and are referred by a physician, case manager or discharge planner. Patients and their families can request a Catholic Health subacute facility by telling their hospital discharge planner of their preference.
I was very impressed and quite pleased with the immediate response to concerns and questions we had of the professional staff.
Our questions were answered on the spot – responses were immediate or researched and a result given in a short time period.
We were pleased our mother was encouraged to join activities. The request was further assisted by escorting her directly to the activity and helping her settle into a position of comfort. This is important to a person experiencing a number of changes at one time.

My mom was a patient at your facility after a bad fall at home.
She was in bad shape – not eating, very weak, in horrible pain. She just laid in bed and screamed every time she had to move.
Your staff was so good with her. I would especially like to recognize Sean, an orderly, and Tanya, RN.
When she left there, she was walking with a walker and eating better, with her pain managed and a lot stronger. She has said to me many times that she would not hesitate to go back to St. Francis if she ever needed subacute rehab again.
So, thank you and your staff for getting my mom moving again. Her progress at home has been slow but steady.
