Becoming a Patient

Physical Therapy

In most cases, patients can receive industrial rehab and physical therapy without a physician referral. Please call the office to schedule an appointment.

A physician referral is required for patients with:

• Medicare
• Medicaid
• Workers Comp.
• No Fault insurance
