Tour Our Center
As part of the Baby and Me Tobacco Free program, you will receive four prenatal counseling sessions to help you to quit smoking. After the birth of your baby, carbon monoxide monitoring will be performed monthly. If you are smoke-free, you will receive a $25 voucher for diapers. Learn more about this program.

Don't Have Insurance?
If you are uninsured, you may qualify for Family Health Plus or Child Health Plus, which are public health insurance plans for New York State residents.
Fidelis representatives can help you apply for Fidelis in the waiting room on designated days. If Fidelis representatives are not available on the day of your visit, we can refer you to a representative to arrange for a visit. Please call our office for more information.
Contact Your Pharmacy: All prescription refills are requested electronically. Please contact your pharmacy and have them send an e-request to your provider.
Request Your Refill in Advance: Medication will only be refilled during office hours. Plan ahead to ensure that your medication does not run out during the night or on the weekend.
Appointment May Be Needed: Patients who have not been seen recently will be asked to make an appointment before refills are given. Patients requesting some types of medications will require monthly monitoring by appointment and refills will only be prescribed at these appointments.
To protect your personal health inforamtion, the release of medical records requires a signed release. Requests for medical records will be processed only with the proper signatures and the full name and location of the person who is to receive the records.
Forms should be brought to your appointment for the provider to fill out. Forms will be completed within 5-7 business days. Patients dropping off forms in-person may be asked to make an appointment, to verify accuracy of information.
Call 911 (Emergency Medical Services) for any life threatening emergency that requires immediate medical care (inability to breathe, severely choking, unconsciousness, seizure, etc.). For poisonings, call the Poison Control Center (716) 878-7654 or 1-800-222-1222.
If you become ill or are injured after office hours or during a weekend or a holiday, your call to the office will go through our answering service. Please limit calls to emergencies or other urgent problems that cannot wait until morning.
