Subacute Care Facilities

 

Subacute Care Across Western New York

Patients enrolled in a subacute program reside for a short period (28 days, on average) at a designated facility where they receive nursing and therapy care to rebuild their strength and also learn how to better manage their disease or condition.

Facilities

Partners In Rehab provides subacute care in dedicated units within four of our Senior Services facilities.

Partners In Rehab's subacute facilities provide a bridge from hospital to home with an integrated approach to care. A multi-disciplinary team of physicians, nurses and therapists meet each patient's individual needs whether they are medical, rehabilitative, social or emotional. The goal is a quick and safe recovery. We want you to achieve your highest possible level of function and self-sufficiency before returning to your home or community.

Patients may need assistance with recuperation after surgery, management of a cardiac or chronic condition or wound care.
