Obtaining Records

Obtaining Medical Records

To obtain a copy of your medical records, send a written request to the hospital you received care at. Be sure to include the signature of the person whose records are being requested.

Please note that if records are requested from a Catholic Health facility, there may be a fee involved. If the request is sent directly to your Medical Provider there should be no charge.

When making a request to a Catholic Health facility for medical records, please address your request to the Correspondence Secretary in the Health Information Department at the appropriate location:

If you were born in one of our hospitals, your birth records are kept in the Clerk's Office in the municipality in which you were born.
