Be sure to include the signature of the person whose records are being requested. Please note that if records are requested from a Catholic Health facility, there may be a fee involved. If the request is sent directly to your Medical Provider there should be no charge. When making a request to a Catholic Health facility for medical records, please address your request to the Correspondence Secretary in the Health Information Department at the appropriate location: Kenmore Mercy Hospital Mercy Hospital of Buffalo
